To re-register your chaperone licence through us, you must live in a Surrey County Council Borough.
You must complete The NSPCC online training course 'Protecting Children in Entertainment a course for Chaperones' before your registration can be approved. The Child Employment Team will send you the link to complete this once you have submitted your application.
I apply to Surrey County Council for approval as Chaperone for children in entertainment under the Children & Young Persons Act 1963 and the Children (Performances and Activities)(England) Regulations 2014. I declare that the information provided in this application is true to the best of my knowledge. I consent to confidential routine enquiries being carried out by the Disclosure and Barring Service and of Surrey County Council Social Care Records. I understand that I will be required to undertake the NSPCC online training course 'Protecting Children in Entertainment a course for Chaperones' (Fee payable to the NSPCC) and show the certificate to the Child Employment Team, before I can be re- registered. I declare that I will notify Surrey County Council of any change of name, address, any criminal convictions or change in circumstances that may affect my ability to effectively carry out the duties and responsibilities of a chaperone. *

Has your address has changed since you were last registered? *
Has your existing chaperone licence expired? (If you are unsure please contact us on 01483 517838 to check) *
If your chaperone licence has been expired for more than 12 months, we require you to apply as a new chaperone and you should not complete this form. Please go to to complete the new chaperone application form.
If your licence has lapsed for 6 months or more we require you to provide two referees so that we can apply for further references for you.
If your licence has been expired 0-5 months, then you do not need to provide references and can skip ahead to the DBS declaration.
Please name two referees (not family members/partners/partners family) who have known you for more than 2 years. They must know of your experience of looking after children (observed you looking after children) and your suitability to act as a chaperone. They must NOT know you solely in connection with your dancing school or amateur dramatic organisation. Please select if you agree to us contacting them on your behalf.
Disclosure and Barring Service Checks
Chaperones will have regular contact with children so this role is exempt from the Rehabilitation of Offenders Act 1974. Therefore you are required to declare any convictions, cautions, bind-overs or prosecutions you may have, even if they would otherwise be regarded as 'spent' under this Act.
Have you ever been convicted of any criminal offence? *
Under the arrangements introduced for the protection of children, Surrey County Council will check under the Disclosure and Barring Service (DBS) for the existence and content of any criminal record. You will be required to complete a DBS form to enable an Enhanced check to be made.
If you are signed up to the DBS Update Service and have an enhanced children's workforce and children's barred list DBS check, if you provide permission we may be able to use this service instead of running a new DBS check. Please contact the Child Employment Team for further information.
All chaperones who have been paid on any occasion, other than out of pocket expenses, cannot be classed as a 'volunteer'. We must therefore make a professional DBS check for you, which will incur an additional charge of £40 & £25 administration fee, i.e. an extra £65. If you declare you are, or intend to be, a professional chaperone then we will send you a link to make the payment by debit or credit card. Alternatively it can be collected when you supply your DBS documents. If you already have a professional DBS check on the update service and your DBS check meets our requirements, you may not need to pay this fee. Please call us to check before doing so. Please confirm below whether or not you have been paid as a chaperone, other than out of pocket expenses.
Have you been employed professionally as a chaperone, receiving payment other than expenses? *
You will need to make an appointment with a member of the Child Employment team to present your 3 identity documents for the DBS check and your online training certificate. We will send you the DBS Information and Accepted Document list for you to see what identity documents we accept. Copies of your documents are not acceptable, we must see the originals.
