Your Ceremony Booking
The name of the hotel, country house, racecourse etc. that you have booked
Name of room and gazebo if it may be outside
Please note that the total number that the room is licensed for must not be exceeded and that children over two years must be counted as individual guests.
Your details
Your full names will be used in the promises but shortened names may be used during the rest of the ceremony, if you prefer.
If you have provided an email address, a copy will be emailed to you once you have submitted the checklist.
Before the ceremony
Would you like to be interviewed together or separately prior to the ceremony? *
If you would like to have your pre-ceremony interview separately, we will do our best to ensure you do not see each other before the ceremony but this cannot always be guaranteed.
How are you entering the room? *
Your ceremony choices
You have a choice of three Surrey ceremonies to choose from. These ceremonies cannot be altered or interchanged and we will not use scripts provided by anyone else. If you would like us to send you a copy of the ceremony scripts, please email registration.ceremonies@surreycc.gov.uk
Which ceremony script would you like the Registrar to use? *
Partnership Promises
The legal Civil Partnership declaration forms part of the Civil Partnership Schedule document. You may choose to read the declaration only or say the declaration to each other in the presence of your guests (in the same manner as marriage vows). In addition, you may choose to exchange promises to each other - please see your civil partnership ceremony pack for the choice of words.
How would you like to exchange declarations? *
Do you wish to write any additional personal promises? *
Any additional personal promises must be solemn and dignified and short in length and must not contain any religious content. The Registrar is not able to read these for you. Personal promises must be approved by the Superintendent Registrar prior to the ceremony. Please email personal promises to registration.ceremonies@surreycc.gov.uk at least 6 weeks before your ceremony. If the personal promises have not been approved before the day of your celebration, they cannot be included in your ceremony.
Are you exchanging rings? *
Witnesses
On the day of your ceremony, you must provide 2 witnesses who are able to speak and understand English. These may be relatives, friends or colleagues and they will be required to sign the Civil Partnership schedule. The Registration Service cannot provide witnesses for you.
If your witnesses change, please let the Registrar know on the day of your ceremony.
Readings - must not have any religious content or be taken from a religious book
Any reading must be solemn and dignified and short in length and must not contain any religious content. Readings must be approved by the Superintendent Registrar prior to the ceremony. If your reading is not from our list of readings, please email to registration.ceremonies@surreycc.gov.uk at least 6 weeks before the ceremony. If the readings have not been approved before your ceremony day, cannot be included in your ceremony.
Would you like to include a reading? *
Music - must not have any religious content or have been written for a religious purpose
Any music content must be approved by the Superintendent Registrar prior to your ceremony. If any music has not been approved before the ceremony day, it will not be included in your ceremony. The Surrey Registration Service must approve any live music requests prior to the ceremony day.

Further information for your ceremony
Are you having a photographer? *
The Registrar will make an announcement before the ceremony starts to advise your guests that photographs cannot be taken during the ceremony.
Would you like your guests to take photos when you enter the room? *
Would you like your guests to take photos after the schedule has been signed? *
Are you having a videographer? *

When you are formally pronounced civil partners, would you like to be invited to share a kiss at the end of the ceremony? *

Terms and Conditions
The couple forming a civil partnership must read and accept our ceremony Terms and Conditions. We wish your day to run as smoothly as possible, so we recommend that you pay particular attention to the contents section of the Terms and Conditions which explains what can be included in your celebration and the timings section which outlines what changes may become necessary to your ceremony if you arrive late.
