If you are renewing a DRIVERS authorised identity badge, please ensure you forward your valid Hackney Carriage / Private Hire license to tcc.authorisedid@surreycc.gov.uk when you submit this form.

 
The data you have provided to Surrey County Council will only be used for the purpose of assessing, issuing and administering a TCC Authorised Identity Badge for use on TCC issued home to school transport contracts. All conditions of the Government Data Protection Regulation Act will be met by the TCC with regards the collection, storage and distribution of your personal data.
